Comments: 

This is one of the limited amount of Tricholomas in the Midwest that prefer hardwoods over conifers. The cap is white, often developing some brownish stains. The stem is also white. There is a final feature that makes it easy to confirm your ID. Both the cap and stem have a tendency to stain blue. Not all specimens present the blue staining, but if yours does, it makes for an easy ID.

The notched gill attachment is what should start you off in the Tricholomas.
